9-How to order testing through your physician
Allermetrix is a reference laboratory and requires a physician's signature to perform testing.
You can print out the request form below and bring it to your physician; M.D., D.O.,
Chiropractor or Naturopath (if they are licensed to order laboratory testing in their state). We
will provide your physician with supplies and prepaid priority mailer upon request.
As part of the CARES act Allermetrix is in network with all insurance companies for
COVID-19 antibody testing. If you have insurance questions, please call.
Alternatively, you can have your physician, or their staff contact us at allermetrix.com or
877-992-4100 for testing supplies. We have two Ph.D. immunologists (owners) that are
available to discuss testing strategies and answer technical questions.
